Vera contacts Julie and Dennis to tell them how grateful she is to have their daughter's heart. But the more time Vera spends with Julie and Dennis, the more she begins to cross boundaries. When Julie and Dennis ask Vera to leave, someone mysteriously ransacks their home and vandalizes their car. Determined to prove Vera is the culprit, Dennis and Julie investigate Vera's past and discover that her sister was killed in the same accident as their daughter, who was reading text messages from Julie while driving. Now, Vera is out for revenge and wants to make Julie pay with her life.

The Black-Hearted Killer is a drama about a middle-class couple, Julie and Dennis, who lost their daughter in an accident. In grief, they decided to donate their daughter's heart. Shortly after the donation was successful, a woman calling herself Vera suddenly came to their door and informed them that she was the one who had received their daughter's heart. Out of memory for her daughter, Julie showed great kindness to Vera and became friends with Vera. However, as time went by, Vera's words and deeds became more and more inappropriate. Dennis warned Vera about her behavior and wanted to draw a clear line with her. After that, strange things happened frequently, and Julie and Dennis began to doubt Vera's true identity. After investigation, they discovered that they knew nothing about Vera at all, and Vera's motive for approaching them was actually related to their dead daughter. …After the tragic death of their daughter, Julie and Dennis decide to donate her organs.
