剧情梗概：
莎拉（肖娜麦克唐纳饰）本来平静幸福的生活被一次车祸意外打破，丈夫和女儿在事故中丧生，她虽然活了下来却从此一蹶不振，为了让莎拉重新振作起来，她的五个女性朋友为她安排了一次探险旅行，地点是在一个丛林中荒僻而古老洞穴，茹诺(娜塔丽杰克逊门多萨饰)是其中最有野外探险经验也是她选择来到这里。正当女孩们对山洞中景色惊叹不已时，却在不知不觉中惊扰了洞中另外一种生物，他们没有视觉，依靠声音的来源残忍的捕杀她们，女孩们为了自救团结起来，却在逃生中朋友间互生怀疑，为了生存下去，她们要如何在狭小黑暗的洞穴中找到出去的路呢？一次意外的车祸，莎拉的丈夫保罗和女儿杰西被一辆货车上飞射过来的尖锐钢管刺穿死去。虽然莎拉侥幸活下来，但自己却从此一蹶不振。一年后，为了让莎拉重返生活正轨，她的5位都刚20岁出头的喜欢户外冒险的朋友们，决定邀她参加一年一度的户外冒险，这次的目的地，是位于美国阿巴拉契亚山脉的查图嘎国家公园的博莱翰岩洞群。她们进入了洞穴，莎拉在一次进入狭窄洞穴的时候被卡住，随后发生了岩洞崩塌，堵住了回路。她们只好向前走，而朱诺也向大家坦白了这里并非是博莱翰岩洞群，而是一个无人发现的新的岩洞。她们继续向前走，却发现这里到处都是动物的尸骨，而远处也能闻声希罕的声音。荷莉在寻找出口的时候，意外地摔断了腿。她被大家扶起继续前进。而此时那些隐藏在洞穴里面的远古类人怪物出现了，他们一下子就把荷莉的脑袋拧断。在尖叫声中，其余的五人慌忙逃窜。朱诺表现得非常的勇敢，她和那些怪物进行搏斗。但是她却意外地杀死了自己的同伴贝丝。她没有管被刺伤脖子的贝丝，而是继续寻找队伍。朱诺找到了丽贝卡和珊姆。其实，她们本可以逃出去的，因为朱诺发现了洞穴出去的标记。但是，朱诺执意要找到莎拉再走。而莎拉却在洞穴中发现了奄奄一息的贝丝。贝丝在临死前告诉莎拉，朱诺曾和保罗有染而且她自己也是被朱诺害死的。新仇旧恨交织在一起令莎拉很是恼火。随后，莎拉找到了朱诺，但是丽贝卡和珊姆却被那些怪物杀死了。当莎拉和朱诺与那群怪物搏斗后，莎拉刺伤了朱诺的一条腿，莎拉将朱诺留给了其余的怪物。而她自己却在慌乱中跌下深坑。当莎拉醒来后，发现有一条堆满白骨的斜坡通道，通道可以看到阳光，她疯狂地爬上去，逃离洞穴，开车一路下山，看到运木材的货柜车，而后惊见朱诺的鬼魂。原来这仅仅是莎拉的梦而已，莎拉没有获救而是仍然在那座岩洞里。莎拉的心灵堕落于黑暗的洞穴中，幻想中见到死去的女儿和生日蛋糕，她会心一笑，忘记了惧怕，沉浸在和女儿的快乐回忆中，无数饥饿的怪物正从四面八方向她爬来，吼叫声不绝于耳。
